Clinical Program Director - PMHNP Program
Sarah Freiberg currently serves in the role of Clinical Program Director for the Psychiatric Mental Health Track of CHC’s NP Residency Program. Sarah was CHC’s first PMHNP resident in 2015 and has been with the health center ever since. She practices in Meriden, and precepts PMHNP residents across the state. Sarah received her Master of Science in Nursing from the Yale School of Nursing, and is certified as a Psychiatric Mental Health Nurse Practitioner. She has worked in various mental healthcare areas including inpatient, in-home and outpatient settings. Her passion lies in providing outpatient community health psychiatric care where she can build and maintain therapeutic relationships with clients over the years and serve as a consistent support, and in supporting new PMHNPs entering the field as they navigate the clinical and systemic complexities to grow confidence in their role and build a sustainable, rewarding career.
